What Your Fee Reflects
Fees are determined by the complexity of your treatment, the materials used, and the duration of care. Crowns from our in-house laboratory have lasted 35 years and longer with proper care, while mainstream patient guidance puts typical crown life at 5 to 15 years (Cleveland Clinic).
Using Your Dental Insurance
Elite Prosthetic Dentistry is a fee-for-service practice. We’re not in-network with any dental insurance carrier and don’t participate in Medicaid or Medicare dental plans, so every patient pays the practice directly. If your PPO plan includes out-of-network benefits, we’re happy to submit claims at no cost or provide you with the necessary paperwork to self-submit.
For most restorative procedures, you’ll receive a two-part form at the end of your visit: your payment receipt, and a form to send to your insurance company. For more extensive procedures, a predetermination of benefits is often submitted before treatment to estimate insurance coverage and help you make a financially informed decision.
Why We Work Outside Insurance Networks
Working outside of insurance networks allows treatment to be planned around your clinical needs rather than plan limitations. Insurance can still help: many PPO plans reimburse part of the cost of out-of-network care, and we provide what your plan needs to process the claim.

Insurance and Financing Questions
Do you accept dental insurance?
We are a fee-for-service practice and are not in-network with any dental insurance plans. However, we're happy to submit the claim for you at no cost or provide the claim documentation so you can request reimbursement directly from your PPO insurance provider.
What does "fee-for-service" mean?
A fee-for-service practice means we prioritize treatment decisions based solely on your clinical needs, not insurance limitations. You pay directly for the care you receive, and we assist you in seeking reimbursement from your plan, if applicable.
Can I use my PPO insurance for out-of-network benefits?
Yes. Many PPO plans offer out-of-network benefits. Because we're out of network, your plan doesn't pay the practice directly, but we're happy to submit the claim for you at no cost or give you the forms and documentation to submit it yourself for possible reimbursement.
Do you accept Medicare or Medicaid?
No. We are not enrolled with Medicaid and do not participate in Medicare or Medicare Advantage dental plans, so we can't bill those plans for your care. Original Medicare doesn't cover routine dental care. You're welcome to be seen as a private-pay patient, and financing is available through CareCredit, Happen Bank (formerly LendingClub Patient Solutions), and Proceed Finance.

For eligible care, yes. Dental treatment that treats or prevents disease, including crowns, implants, and dentures, is generally an eligible HSA and FSA expense, while purely cosmetic procedures such as teeth whitening are not. Your plan administrator can confirm what your account covers.
