Smile Makeover with Dentition Reconstruction

The Challenge

Our patient from Bethesda, Maryland was referred by her Periodontist to Elite Prosthetic Dentistry for comprehensive reconstruction of her upper dentition. She faced a complex clinical situation: previously placed dental implants in her upper left jaw had failed, and with little available bone in that part of her upper jaw, she was not a candidate to receive replacement implants. She wanted to have a fixed restoration replacing many teeth, which presented a significant clinical challenge. The fundamental question was how to stabilize all of her upper teeth to restore both function and aesthetics while making the final result appear completely natural.

Treatment Planning and Design

We developed a comprehensive treatment plan involving custom temporary crowns to be fabricated and fitted to her remaining tooth abutments. These temporary crowns were designed to blend perfectly with her facial pattern and lip line, accounting for her unique smile proportions. Pink acrylic was used to simulate her missing gingival (gum) tissue, creating a natural-looking transition between the tooth-colored crowns and gum-colored acrylic. An immediate smile makeover was created on the day she received the crowns as they were custom fitted to her tooth abutments.

Treatment Approach and Technical Considerations

To achieve the best results, our in-house laboratory team focused on hand-crafting a perfectly fitting underlying framework that would serve as a rock-solid foundation for all the crowns. We did not use milled frameworks, as they cannot provide the same level of precision fit that is essential for long-term success in complex cases like this. The framework design was custom engineered to properly distribute forces and ensure long-term stability.

Her porcelain crowns were meticulously hand-crafted using internal coloration and multiple translucent porcelains to blend perfectly with her complexion. The color gradations and translucency characteristics were carefully matched to her natural tooth structure. Her gingiva was naturally simulated using pink porcelain, creating a seamless transition that makes the restoration appear as natural as possible.

Results and Next Phases

An immediate smile makeover was created on the day she received her custom-fitted temporary crowns, dramatically improving her appearance and confidence. The success of this case demonstrates the importance of custom hand-crafted restorations in cases with multiple missing teeth.

She is now prepared to undergo the next phase of treatment with lower anterior and left crowns, which will create a comprehensive full-mouth reconstruction that restores both function and aesthetics.
