Salvaging a Severely Compromised Dentition

When a patient presents with multiple missing teeth, severe decay affecting many remaining teeth, and a collapsed bite vertical dimension, the situation can feel hopeless. Many patients in this situation resign themselves to wearing a denture, assuming that tooth loss and extraction are inevitable. However, modern prosthodontic reconstruction techniques, combined with careful treatment planning and patient commitment, can often salvage the natural dentition and restore function and aesthetics without requiring a removable appliance.
The Challenge: Comprehensive Dental Failure
This patient presented with a complex clinical situation. She had several missing teeth, many remaining teeth were severely decayed with extensive loss of tooth structure, and the vertical dimension of her bite had collapsed dramatically. The collapse of her bite was aging her appearance prematurely and creating an artificial, sunken facial profile.
Most critically, she was in danger of losing her remaining upper teeth entirely. Without intervention, extraction and complete denture therapy would likely be her only option. The patient was understandably distressed and seeking any solution that would allow her to retain her natural teeth and avoid the challenges of denture wear.
The treatment challenge was to develop a comprehensive plan that would stabilize her remaining teeth, address the collapsed bite, restore proper dental proportions, and improve her overall appearance and function, all while preserving her natural teeth when possible.
Treatment Approach
Dr. Gerald Marlin conducted a comprehensive evaluation of the patient’s remaining tooth structure, bone support, and overall oral health. Rather than immediately recommending extraction and implant placement, he developed a salvage plan designed to preserve her remaining natural teeth when clinically appropriate.
The primary treatment strategy involved opening her vertical bite dimension. The collapsed bite was not only cosmetically aging but also created mechanical disadvantages for chewing function and tooth support. By increasing the vertical dimension, Dr. Marlin could restore more natural proportions to her smile and improve the mechanical advantage for the remaining teeth.
Strategic tooth reconstruction was performed on the teeth selected for preservation. Severely decayed teeth were carefully prepared to remove all diseased structure, and the remaining tooth anatomy was meticulously rebuilt to restore proper contours and support for new restorations. This careful planning allowed maximum tooth preservation while ensuring long-term success.
New custom-crafted crowns were fabricated that reflected the improved vertical dimension and created natural, attractive proportions that matched her facial features and lip line. Our in-house laboratory designed each crown with specific attention to creating restorations that would support her newly opened bite while appearing natural and youthful.
The result was comprehensive mouth rehabilitation that preserved her remaining natural teeth while restoring full function and improved aesthetics. The patient was able to avoid a denture and maintain her natural dentition with fully functional restorations.
Results
The transformation was profound. The patient’s appearance was dramatically improved by opening her bite and creating restorations that reflected natural tooth proportions and color. Her previously collapsed profile was restored, and her facial appearance became years younger.
More importantly, she successfully avoided tooth extraction and the challenges of denture wear. She retained her natural teeth with improved function, health, and longevity. The restored bite improved her chewing function, reduced mechanical stress on her remaining teeth, and provided a foundation for long-term oral health.
This case demonstrates that even severely compromised dentitions can often be salvaged through expert prosthodontic reconstruction. The key is comprehensive evaluation, strategic treatment planning, and skilled execution of complex restorative procedures.
The Value of Specialized Expertise in Complex Cases
When you have multiple missing teeth, severe decay, and a compromised bite, the stakes are high. A general dentist may recommend extraction and dentures as the simplest solution.
