Restoring multiple missing teeth in a single quadrant of the mouth using dental implants is a complex undertaking that requires careful surgical and prosthodontic planning. When patients have sustained bone loss from missing teeth, the challenge becomes even more complex. However, with modern bone grafting techniques and expert implant placement, even severely deficient bone can be rebuilt to support a full arch of fixed implant crowns.

The Challenge: Bone Deficiency and Multiple Missing Teeth

This patient presented with the need to restore the upper left quadrant, where several teeth were missing and bone support was inadequate for implant placement. With so few remaining teeth in the quadrant, she faced a difficult choice: accept a removable partial denture with its inherent limitations of maintenance, comfort, and function, or pursue a more complex implant solution that would require bone grafting to provide adequate support.

The critical challenge in this case was the presence of a very large maxillary sinus. The sinus pneumatized downward into the bone where teeth had been missing for an extended period, leaving virtually no bone support in the left posterior quadrant. Without bone, implant placement would not be possible, and she would be relegated to removable prosthodontics.

However, the patient understood the limitations of denture wear and desired a fixed solution that would provide complete function and stability without relying on a removable appliance.

Treatment Approach: Bone Grafting and Strategic Implant Placement

Dr. Gerald Marlin developed a comprehensive treatment plan involving two major surgical phases followed by prosthodontic restoration.

The first phase involved bone grafting of the left sinus area. Using bone augmentation techniques, Dr. Marlin worked to rebuild the bone volume in the posterior maxilla, effectively raising the sinus floor and creating adequate bone volume to support implant placement. This is a sophisticated surgical procedure requiring expertise in sinus anatomy and bone grafting principles.

After the grafted bone had healed and integrated with the patient’s native bone, the second surgical phase involved placement of dental implants to serve as artificial tooth roots in the reconstructed bone. Multiple implants were strategically positioned to provide optimal support and distribution of forces across the restored quadrant.

The final prosthodontic phase involved fabrication of custom dental crowns on each implant. The team worked to create restorations that would be functionally optimal and aesthetically natural-appearing, seamlessly integrating with her remaining natural teeth.

Results

The patient achieved complete restoration of function in her left quadrant through a full arch of fixed implant crowns. More importantly, she was able to avoid wearing a removable denture, which had been her primary concern. The fixed restoration provides superior comfort, chewing function, and long-term stability compared to removable prosthodontics.

The restored quadrant now functions as if she never lost the teeth. She can eat normally, speak clearly, and smile with complete confidence. The implant-supported crowns are stable, durable, and designed for long-term success.

This case demonstrates that even when bone is severely deficient, modern bone grafting combined with expert implant placement can provide fixed restorative solutions that significantly improve quality of life.
