Understanding the Dire Situation: Massive Bone Loss

Lucy, a patient from Bethesda, Maryland, presented with one of the most challenging dental situations that can confront a prosthodontist. She had experienced the complete loss of bone structure in her upper left anterior region, extending all the way up to the floor of her nose. This catastrophic bone loss occurred as a consequence of the surgical extraction of her impacted upper left canine (#11).

When teeth are impacted, meaning they have failed to erupt normally through the gums, their extraction can result in significant bone loss. In Lucy’s case, the surgical trauma combined with the years following the extraction resulted in such extensive bone resorption that her dental anatomy had been fundamentally altered. This situation presented multiple challenges for restoration.

Additional Complications: Multiple Missing Teeth and Compromised Dentition

Lucy’s situation was further complicated by the fact that she was missing multiple teeth adjacent to the surgical defect. Additionally, several of her other teeth were broken down and severely compromised. These problems combined to create a situation where traditional tooth replacement with simple crowns and bridges would not be adequate.

Lucy’s bite was also non-functional, meaning she could not bite and chew properly. Her teeth were discolored and uneven, and her facial appearance had been compromised by the loss of dental support and structure. For many patients in Lucy’s situation, a prosthodontist might recommend a complex multi-stage surgical and restorative approach extending over several years.

The Challenge: Implant Placement in a Severely Deficient Site

The key challenge in Lucy’s case was that the area where her teeth needed to be restored had virtually no bone remaining. To place dental implants, adequate bone structure must be present to support them. This required careful surgical planning to place the implants within the severely constricted space and bone anatomy that remained.

Elite Prosthetic Dentistry was able to develop a plan for two preplanned dental implants that would need to be successfully inserted within this extremely limited surgical space. The implants would serve as anchors for comprehensive restorations that would restore both function and appearance to Lucy’s smile.

The Solution: Strategic Implant Placement and Restoration

Working with a coordinated surgical team, Dr. Marlin was able to place the two dental implants in carefully planned positions that would provide adequate support for the final restorations. The implants were positioned to maximize success and longevity despite the challenging anatomy.

Once the implants had successfully integrated with the remaining bone, the restorative phase began. Custom crowns were fabricated and cemented to the implant abutments, restoring missing teeth and creating a natural, functional bite.

Results: Complete Functional and Aesthetic Restoration

After placing the dental implants, Lucy was given a new smile with complete function and very good long-term prognosis. The comprehensive restoration addressed all of her dental problems simultaneously. Her bite was now functional, allowing her to bite and chew properly. Her teeth were no longer discolored and uneven, they were bright and beautifully aligned.

Needless to say, Lucy is thrilled with her rejuvenated mouth and smile. The transformation has restored her confidence, her appearance, and her ability to function normally.
