Avoiding Dentures Through Strategic Implant-Supported Restoration

When all of the upper teeth are failing due to decay, damage, or periodontal disease, patients often resign themselves to wearing a denture, accepting this as an inevitable consequence of tooth loss. However, modern dental implant therapy has transformed the possibilities for full arch reconstruction. Rather than accepting a removable denture with its inherent limitations, patients can now achieve fixed restorations that provide superior function, esthetics, and quality of life.

The Challenge: Complete Maxillary Failure and Denture Risk

This patient presented with a critical clinical situation. All of her maxillary teeth were failing and showing signs of advanced decay, mobility, or structural compromise. Without intervention, she would inevitably require extraction and would face the prospect of wearing a denture either during the transition to implant treatment or permanently.

The psychological and functional implications of denture wear are significant. Dentures require daily removal and cleaning, have variable retention and stability, can affect speech clarity, limit food choices, and often create feelings of social self-consciousness. Most patients facing this situation are desperate to find an alternative.

The treatment challenge was to develop a comprehensive plan that would allow extraction of the failing teeth while maintaining the patient out of a denture during the treatment and healing phases, and ultimately providing fixed implant-supported restorations that would function and feel like natural teeth.

Treatment Approach: Interim Retention and Implant Planning

Dr. Gerald Marlin developed a sophisticated treatment plan with several coordinated phases designed to avoid denture wear entirely.

The initial phase involved careful evaluation of which teeth might be salvageable on an interim basis. Even though many of the patient’s teeth were compromised, a few teeth with reasonable structure and support were identified as candidates for interim restoration through custom-crafted crowns.

These interim restorations kept the patient out of a denture during the treatment period. Having even a few retained natural teeth provides significantly better function, esthetics, and psychological benefit compared to denture wear during the treatment transition.

As the interim crowns were being completed, dental implants were strategically placed to provide support for a full arch of fixed restorations. Seven implants were positioned to provide optimal support distribution and long-term stability.

After implant osseointegration was complete, a comprehensive full arch of custom-crafted crowns was fabricated on the implants. The crowns were designed to provide optimal function, natural appearance, and support for healthy gingival tissue.

Results

The patient was completely reborn with a full arch of crowns seated on seven implants. She avoided denture wear entirely during the treatment process and now has permanent fixed restorations that provide complete function and esthetics.

The transformation in her quality of life was profound. She has newfound confidence in her smile, the ability to eat all foods, clear speech, and a sense of security that dentures could never provide. She is thrilled with both the functional and esthetic improvements and deeply appreciates that the strategic treatment planning allowed her to avoid denture wear.
