Not all cosmetic smile improvements require the same approach. Sometimes a patient presents with specific clinical situations that demand careful evaluation to select the optimal restoration type. The Challenge

This patient was referred by her plastic surgeon seeking cosmetic improvement through a new set of porcelain veneers on her four front teeth. She had existing composite veneers that needed replacement, and she expected a straightforward restoration with new porcelain veneers.

However, when Dr. Gerald Marlin conducted a comprehensive examination, a significant clinical challenge emerged. Upon careful evaluation, he discovered that the patient had very large existing fillings underneath the old veneers on her incisors. This created a critical issue: veneers are thin restorations that are bonded to tooth structure, and they require sufficient sound tooth material to be successful. With such large underlying fillings, veneers would be contraindicated because the risk of future decay beneath the restorations would be high, potentially compromising the veneers and requiring replacement.

The challenge was to achieve the aesthetic improvement the patient desired while selecting a restoration type that would provide superior longevity and protect the underlying tooth structure with large fillings.

Treatment Approach

Rather than proceeding with veneers as originally planned, Dr. Marlin recommended custom-crafted crowns as the optimal solution. While crowns require more tooth preparation than veneers, they provide superior coverage and protection for teeth with large existing restorations. More importantly, crowns can be fabricated with internal shading and advanced translucency techniques that rival or exceed the aesthetic potential of veneers.

Our in-house laboratory designed the crowns with careful attention to achieving the aesthetic goals the patient originally sought. The crowns were internally shaded to create depth and dimension, with carefully selected translucency levels to provide a vibrant, natural appearance. Rather than creating flat, opaque restorations, the team engineered each crown to have subtle color graduation and light transmission properties that mimic natural tooth structure.

The patient was involved throughout the treatment planning process, and she understood that this modified approach would provide superior long-term outcomes and aesthetics that she would love even more than traditional veneers.

Results

The patient was thrilled with her results. The custom-crafted crowns achieved the vibrant, natural appearance she sought, with exceptional translucency and depth that creates a 3-dimensional, lifelike look. Because the crowns provide complete coverage, the underlying large fillings are protected from future decay and degradation.
