Many people in the U.S. suffer from extensive periodontal disease characterized by significant bone loss and shrinkage of the gum tissue. This condition can begin at a very young age and worsen quickly due to hereditary factors and lack of early diagnosis by their dentist. As teeth are lost and bone shrinks, the smile line becomes increasingly unsightly, often resulting in loss of confidence and social withdrawal.

Ms. N presented to us in complete distress with two lower anterior teeth that were about to fall out and wide gaps opening between her upper and lower anterior teeth. At just 34 years old, she was already experiencing severe periodontal disease that had devastated her smile and self-esteem. She was unable to smile or speak with confidence and was desperate to find a solution that would restore both function and appearance to her smile.

Treatment Approach

Ms. N was in need of comprehensive periodontal prosthesis therapy, the restoration of the adult dentition in the presence of bone loss and gum shrinkage caused by periodontal disease. This is an advanced and complex treatment that requires careful planning and coordination between the prosthodontist and the patient’s periodontal status.

To provide immediate relief and improvement to her smile while planning for long-term solutions, we fabricated custom temporary splints to stabilize her remaining teeth and fill in the gaps between them. This provided an immediate smile makeover that dramatically improved her appearance while we planned the next phase of treatment.

In two separate all-day visits under dental sedation, we extracted the hopeless teeth and placed bone grafts to prevent future bone loss and provide a more stable foundation for future restorations. Following the initial treatment and healing period, we inserted custom-crafted temporary crowns using porcelain and pink acrylic to simulate her missing gingival tissue and create a natural-looking smile.

The comprehensive treatment plan addressed not just the immediate aesthetic concerns but also the underlying bone loss that would have continued to worsen without intervention. Our in-house laboratory team worked closely with Dr. Marlin to create restorations that were both beautiful and functional.

Results

Ms. N received an immediate and dramatic smile transformation, allowing her to smile with confidence once again. The bone grafts were placed strategically to preserve remaining bone and prevent further deterioration. The temporary crowns restored her ability to speak and smile naturally, and they provided a template for her final restorations.

Long-Term Care and Monitoring

Ms. N is thrilled with her new appearance and the security of knowing that we will be constantly monitoring her periodontal condition. Since her predisposition to periodontal breakdown could cause problems at any point in time, ongoing professional monitoring and excellent home care are essential to maintaining her results. This case demonstrates how a comprehensive approach, combining extraction, grafting, and custom prosthetics, can salvage a severely compromised smile.
